Evaluation Results: Peyton accompanied this examiner to the evaluation without visible signs of distress. He was pleasant and cooperative and participated well in the evaluation process. His spontaneous speech contains numerous sound substitution and omissions errors which make intelligibility difficult without context clues. Peyton's testing behavior is judged by this examiner to be adequate for his age.
Social-Emotional: The DAYC Social-Emotional was completed to assess Peyton's social-emotional skills. Results of this procedure indicate that Peyton's social-emotional skills are within the higher-than-average range with a standard score of 121.
Curriculum-Based Assessment: The Child Development Checklist was completed to determine Peyton's performace in the areas of gross motor, social and play, pre-academic, oral language, language comprehension, music/story, self-help, and fine motor. Results of this procedure indicate that Peyton is performing 100% of the gross motor skills; 100% of the social and play skills, 100% of the pre-academic skills; 100% of the oral language skills; 100% of the language comprehension skills; 100% of the music/story/art skills; 100% of the self-help skills; and 67% of the fine motor skills.
Oral Peripheral Examination: A visual inspection of Peyton's lips, dentition, tongue, and palates was attempted to asses adequacy for speech production. Results of this procedure reveal a judgement of normal structure and function of the oral mechanism for the purpose of speech production. Limited visualization of Peyton's tonsils occured due to obscurity of his tongue.
Informal Assessment of Voice: An assessment of Peyton's voice quality, resonance, pitch, and intensity was performed during conversational discourse. Results of this procedure reveal a judgement of normal voice for Peyton's age and gender.
Informal Assessment of Fluency: An informal assessment of Peyton's speech fluency was also performed during conversational discourse. Results of this informal procedure indicate a judgement of normal rate and rhythm with no dysfluencies noted to occur.
SPEECH: The GOldman-Fristoe Test of Articulation:2: was administered to assess Peyton's production of consonant sounds in initial, medial, and final positions of single words. This assessment procedure yields a standard score, percentile range, and age equivalent. Standard scores are based on a mean of 100 and a standard deviation of 15.

The HAPP-3 was administered to assess Peyton's speech for phonological deviations. The HAPP-3 yields severity ratings (i.e., normal, mild, moderate, severe, and profound) based on the number of phonological deviations and the percentage of phonological deviations produced. This assessment procedure utilizes objects/pictures for the purpose of eliciting spontaneous productions. HAPP-3 guidelines suggest the phonological deviations occuring greater than 40 percent should be targeted for remediation.

This assessment procedure yields standard scores, percentile ranks, and age equivalents. Standard scores are based on a mean of 100 with a standard deviation of 15. Spontaneous productions are elicited via picure stimuli. Based on Peyton's responses, the following results were obtained:

This assessment procedure yields standard scores, percentile ranks, and age equivalents. Standard scores are based on a mean of 100 with a standard deviation of 15. Language strengths and weakness can be determined based on the child's chronological age.

The RWOPVT yields standard scored, percentile ranks, and age equivalents. Standard scores are based on a mean of 100 with a standard deviation of 15. Based on Peyton's responses, the following results were obtained:
